Coffee Cuts Liver Cancer Risk 47% in Massive 13-Year Study

Coffee liver cancer risk reduction is the headline finding of one of the largest liver-health studies ever conducted: adults who drank five or more cups of coffee a day had a 47 percent lower risk of liver cancer, a 32 percent lower risk of cirrhosis, and a 42 percent lower risk of dying from liver disease. The research, led by investigators at Cedars-Sinai and published this month in Clinical Gastroenterology and Hepatology, tracked nearly 355,000 adults for a median of 13 years.
The study drew on the UK Biobank, one of the world’s richest health databases, enrolling participants who had no history of cirrhosis or liver cancer at the outset. Researchers then followed their health records for more than a decade, comparing outcomes across coffee consumption levels — from complete abstainers to heavy drinkers of five-plus cups daily.
What makes the findings especially persuasive to hepatologists is the dose-response relationship: protection increased steadily with each additional cup. Even moderate drinkers saw meaningful benefits — participants consuming just one to two cups daily showed a 20 percent lower risk of developing cirrhosis and a 31 percent lower risk of liver-related death compared with non-drinkers.
The protection was not limited to caffeine devotees. Both caffeinated and decaffeinated coffee were associated with healthier livers, suggesting the benefit comes from coffee’s hundreds of other bioactive compounds — including chlorogenic acids, kahweol, and cafestol — rather than caffeine itself. That finding matters for the millions of people who avoid caffeine for cardiac, sleep, or anxiety reasons.
The researchers went beyond disease statistics. MRI scans of participants revealed that coffee drinkers had measurably less liver fat, inflammation, and scarring, while blood tests showed healthier liver-function markers and protein patterns. In other words, the study did not just count diagnoses — it captured biological evidence of healthier liver tissue in coffee drinkers years before disease would appear.
Liver disease is a quiet epidemic. Metabolic dysfunction-associated steatotic liver disease, formerly known as fatty liver disease, now affects roughly one in three adults worldwide, driven by obesity and diabetes. Cirrhosis and liver cancer kill more than two million people globally each year, and liver cancer remains one of the deadliest and fastest-growing cancers in the United States. A cheap, widely consumed beverage that lowers those risks is a public-health finding with enormous reach.
The results reinforce a body of evidence that has been building for two decades. Previous research linked coffee to lower rates of liver fibrosis and hepatocellular carcinoma, and the World Health Organization removed coffee from its list of possible carcinogens in 2016, later acknowledging evidence of protective effects on the liver. What the Cedars-Sinai analysis adds is scale, duration, imaging data, and biomarker depth that earlier studies lacked.
Researchers caution that the study is observational — it demonstrates strong association, not airtight causation. Coffee drinkers may differ from abstainers in ways statistics cannot fully capture. The team adjusted for alcohol use, smoking, body mass index, diabetes, and socioeconomic factors, and the associations held firm, but randomized trials would be needed to prove cause and effect definitively.
There are also important caveats about what counts as coffee. The benefits observed were tied to brewed coffee itself, not to sugar-loaded specialty drinks. Hepatologists note that a 500-calorie caramel concoction likely cancels the benefit for a liver already battling metabolic stress, and that coffee is no substitute for the fundamentals: limiting alcohol, maintaining healthy weight, and managing blood sugar.
Reaction from the medical community has been enthusiastic. Gastroenterology societies highlighted the study as further reason to stop advising patients with liver concerns to quit coffee — outdated guidance that persists in some clinics. Several specialists said they now actively encourage two to four daily cups for patients at risk of fatty liver disease, provided caffeine is tolerated.
For consumers, the practical guidance is refreshingly simple. If you already drink coffee, keep drinking it — and an extra cup may help rather than hurt your liver. If you avoid caffeine, decaf appears to deliver similar liver benefits. If you do not drink coffee at all, experts stop short of prescribing it, but the evidence increasingly suggests the world’s most popular beverage is one of the easiest liver-protective habits available.
The takeaway: in a 13-year study of 355,000 adults, coffee drinkers had dramatically lower rates of cirrhosis, liver cancer, and liver-related death, with benefits rising cup by cup and extending to decaf. As liver disease surges worldwide, the humble cup of coffee is looking less like a guilty pleasure and more like preventive medicine.
